Insurance Information for UIU Student-Athletes & Parents
Upper Iowa University requires all university students and student-athletes to have a personal medical insurance policy. It is highly recommended that student-athletes have a policy that includes adequate coverage of athletic injuries and accidents. It will be to the student-athlete’s advantage to contact their insurance provider prior to the upcoming school year and request the following:
1. Medical coverage of intercollegiate athletic injuries, conditions, and accidents
2. Medical coverage in Northeast Iowa and the surrounding area
3. Medical coverage with the following physicians:

The UIU Athletic Department
DOES NOT provide additional athletic insurance coverage. Therefore all medical costs due to athletic injury (including but not limited to: physician services, x-ray, MRI, injections, surgeries, rehabilitation services, etc.) will be the complete responsibility of the student-athlete and their personal medical insurance. Thus, it is imperative to take the recommended steps above to assure each student-athlete is sufficiently covered to receive the most prompt and effective care.

ADHD/ADD
- Student-athletes who have been diagnosed with ADHD/ADD are required by Upper Iowa University to have the following form completed by their physician or health care provider prior to intercollegiate athletic participation.
Student-athletes and parents should be aware that some medications prescribed to treat ADHD/ADD are on the NCAA banned drug list. Therefore, if a student-athlete taking these medications were to be drug tested by the NCAA or Upper Iowa University, it could result in a positive test. By completing the
ADHD/ADD Exempt Form and providing UIU Athletic Training Department with the necessary documentation you may be considered exempt from a positive test for the prescribed medication.
