Upper Iowa University Athletic Training utilized an electronic medical record system to collect all paperwork. This program is called Athletic Training System. Each athlete will have (or has) their own ID and password to get into their profile. Instructions will be sent on how to complete this to the e-mail provided to the athletic trainer. Returning student athletes will use the same profile as the previous year. New student athletes will be provided their log in information from their athletic trainer.
In addition to the electronic forms, a pre-participation physical needs to be completed and signed by a physician. New student athletes to Upper Iowa University must provide proof of sickle cell testing as well.
All forms must be completed by August 1st.
